The US Veteran population is rapidly aging and almost half of all Veterans are aged 65 or older. Delivery of health care services to older adults can be challenging, fragmented, and poorly coordinated, which can lead to duplication of services and care that may not address patient-centered needs. In 2017, the John A. Hartford Foundation, the Institute for Healthcare Improvement (IHI), the American Hospital Association (AHA), and the Catholic Health Association (CHA) of the US jointly launched the Age-Friendly Health System (AFHS) initiative.
