There is growing momentum to incorporate community health workers as integral members of care teams to address health disparities. More data are needed on the impact of robust, real-world models of community health workers when integrated into clinical teams. The Community Health Representative (CHR) Program serves more than 250 American Indian and Alaska Native communities in the United States. This study evaluated the impact of an intervention (Community Outreach and Patient Empowerment [COPE]) that strengthens the Navajo Nation CHR Program through increased coordination between CHRs and clinic-based providers and structured outreach to individuals living with diabetes.
